Death Registration Appointment

By law you must register a death within five calendar days unless the Coroner is investigating.

The death cannot be registered until the doctor or hospital provides a Medical Certificate of Cause of Death or, if the Coroner is involved, the relevant paperwork has been provided to us.

The closest relative MUST register the death (unfortunately statute does not regard an unmarried partner as a relative).

Documents for the relative registering the death, as well as the deceased, must be provided to show names and addresses to prevent any errors.

Deaths occurring at the The Princess Royal Hospital will be registered with us. However if the death occurred at home, or elsewhere in Telford & Wrekin, please check that the exact location is within the district. Deaths must be registered in the district that they occur but as some addresses with a Telford & Wrekin postcode are not included within our district it is best to check.

Once your appointment has been made you will be given a reference number; please keep this safe as it will be required if you need to contact us to amend or cancel your appointment.
