Birth Registration Appointment
Welcome to the Telford & Wrekin Registration Service through which you can book a birth registration appointment at the Register Office in Wellington.
The birth of your baby must be registered within 42 days (6 weeks) and should be registered in the district in which they were born.
(If lead times exceed the desired 42 day registration period, please continue to book the next available appointment)
You must arrive 10 minutes before your appointment. We operate a strict policy whereby, If you are more than 5 minutes late for your allocated time, you may not be seen and will have to rebook for an alternative day.
If your baby was not born within Telford & Wrekin you can still register a birth by declaration but you will not receive a birth certificate at your appointment. To avoid delay we recommend you register the birth in the district in which your baby was born if you are able to do so.

- Please note that all birth certificates are chargeable at a cost of £11 each
- If the mother and father/second parent of the child were married to each other or in a civil partnership at the time of the birth, either parent may register the birth
- If the mother and father/second parent of the child were not married to each other at the time of the birth, the mother alone may register the birth
- If not married to the mother, the father's/second parent's details can only be entered (in most cases) into the register if they also attend the registration appointment
- To complete the birth registration you will be asked to provide proof of parent(s) ID, proof of your address, marriage/civil partnership certificate (if applicable) and the child’s ‘red book’ issued by the hospital

Please note - once you have booked your birth registration appointment you will receive a booking confirmation email and booking reference number. If you do not receive this your booking may not been unsuccessful and we recommend you contact us immediately by telephone or email to confirm your appointment details.
